All athletes in grades 7-12 during the 2011-2012 school year must have a current physical on file at  LCC in order to participate in athletics.  Athletes must have a new physical every year.  Below are links to all the forms that must be completed, signed by a parent/guardian and taken to your physicians office when your son/daugher receives their physical.

Physical Information Letter - This is the letter attached to the packet of information that the athletes can pick up in the general office at LCC.  The letter details why and how the forms are to be completed.

  OIO Consent for Pre-Participation Physical Form - This form must be completed and signed by the parent/guardian and brought with the athlete to the physicals at OIO.

 St. Rita's Medical Information Release Form 2011-2012 - This form must be completed and signed by the parent/guardian.  This form is required because the Athletic Trainer is employed by St. Rita's.

 OHSAA Pre-Participation Physical and OHSAA Authorization Form - Consists of 4 pages.  Page 1 must be completed and signed by parent/guardian, Page 2 is completed, signed and DATED by the Physician, and pages 3 & 4 must be signed by both the student and the parent/guardian.
