FREE Physicals for all LCC athletes entering grades 7-12 for the 2008-2009 shool year will be offered by the Orthopaedic Institute of Ohio on Monday, May 5th and Monday, May 12th from 6:00-8:00 pm.   Below are links to all the forms that must be completed, signed by a parent/guardian and brought to OIO.

 Physical Instructions letter  This is the letter attached to the packet of information that the athletes can pick up in the general office at LCC and will be available at the Junior Highs very soon.  The letter details why how the forms are to be completed.

 2008-2009 OHSAA Physical form  This is the 4 page form that must be brought to OIO for physicals.  Page 1 must be completed and signed by parent/guardian, Page 2 is complete by the Physician, and pages 3 &4 must be signed by both the student and the parent/guardian.  There is a 5th page to this document but it is blank and does not have to be printed (this is an error on the part of the OHSAA).

 OIO Consent for Physical  This form must be signed by a parent/guardian and brought with you to OIO for the physicals...if you do not have this form you will NOT receive a physical.

   2008-2009 SRMC HIPAA form  This form must be completed and signed by the parent/guardian.  This form is required because the Athletic Trainer is employed by St. Rita's.

All athletes in grades 7-12 during the 2008-2009 school year must have a current physical on file at  LCC in order to participate in athletics.  Athletes must have a new physical every year.
